The forms provided/listed below must be completed prior to their
first day at Mill Creek Early Childhood Program.

Please provide the following:

*** A copy of your child's Birth Certificate ***

Please complete/sign the following forms:

Medical Form

Please make sure your child's physician signs and completes the entire medical form.
Please make sure you complete and sign the parent portion of the medical form.
​All children 6 months of age and older will need a medical form that shows that a Lead Risk Questionnaire was completed.
